Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to provide a good faith estimate of expected charges for items and services to individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing, upon request or at the time of scheduling health care items and services.
If you are using insurance, you will not be "balance billed" and will be responsible only for the copayment, coinsurance, or deductible dictated by your insurance company. If you are not using insurance, you will be provided an up front out of pocket rate.
If you are billed for an amount that is at least $400 more (per service) than the total amount on the good faith estimate you got from your health care provider, you (or your authorized representative) may dispute the charges in the Federal patient-provider dispute resolution process. You have to ask for the review within 120 days of getting the bill. An independent reviewer will look at the good faith estimate, the bill, and information from the provider to decide the amount, if any, that you have to pay for each service.

Cancelling an appointment with insufficient notice prevents other clients from being offered a sessopm. There is a 24-hour cancellation policy in effect for all clients of Dr. Burch. It is important to attend your scheduled appointments to ensure continuity of care and to demonstrate commitment to the therapeutic process. If 24 hour notice is not given to Dr. Burch to cancel or reschedule an appointment, or you do not show up for your scheduled appointment, a cancellation fee will be applied to your account. Your insurance does not cover this charge and it is the full responsibility of the patient. Multiple late cancellations or no shows may lead to dismissal from the practice.
